Personalising the booklet

Parents are more likely to value and keep a booklet that they feel has been endorsed by you and personalised for them. For this reason, we encourage you to write on the booklet.

There is a space on the front cover for you to write the child’s name – please do this.

There are boxes next to each section so that you can tick the sections you have discussed.

There are sections for you to write your surgery number and an out of hours contact number on the back page (although you may have completed these prior to using the leaflet).

Please also feel free to highlight or circle things, or write in extra information that you feel is relevant.
